Accurate reporting and testing ensures that your 401(k) Plan remains compliant with both the Department of Labor (DOL) and the Internal Revenue Service (IRS). To complete your annual reporting, please do the following:

1. Download the Year End Summary and Census Form. You will need Adobe Acrobat Reader to view these.
